Self-management programs provide education and tools to help people live healthier and better manage the daily tasks of living with chronic conditions.

This website provides information on self-management and workshops for both the general public and health care professionals so that everyone can be Living Well.

The Living Well workshops have been funded by the Ministry of Health and Long-Term Care and are provided free of charge to participants.
