Do you live with a chronic condition, used to, or care for someone who does? Do you like to help others and are comfortable speaking in front of small groups? If so, we have a great opportunity for you.

We are holding training sessions to become a Peer Leader in the Stanford Chronic Disease Self-Management Program. As a peer leader you would teach our 6-week Living Well with Chronic Disease workshops in our region.

If you review and agree to the expectations and requirements listed in our Peer Leader Agreement Form that you will need to sign after training, we will cover your training costs. We cover the cost of tuition and materials and can also cover the cost of travel if needed (train, bus or $0.50/km if using your own vehicle, and parking). All original receipts are required for reimbursement.

Please note that the that the Stanford program is based on a peer-led program model. Health care providers are welcome to become Leaders, particularly for organizations who intend to run the program regularly, but keep in mind that when facilitating this workshop, the workshop and materials are to be approached from a “peer” perspective vs. a health care professional perspective. This implies a few things:

  • The Stanford Self-Management Program is a well-established and evaluated workshop that demonstrates positive impacts on health outcomes. The materials are copyrighted and are intended to be strictly followed with only minimal paraphrasing and no insertion of additional materials or advice. You will be expected to follow the workshop outlines and content as is.
  • Depending on your background you may or may not already know some of the program content.
  • Even if you already feel comfortable as a presenter/facilitator, you will be expected to practice teaching the content as part of the required exercises/curriculum.
