Providing you with online support to take control of your health
Chronic disease is a significant health care problem in Canada, with many Canadians living with at least one chronic health condition. Individuals living with chronic conditions benefit from an active engagement in their own health care. Patients who have the skills, information, and confidence to better manage the physical and emotional impacts of their illness tend to feel better and use health care services differently, which can optimize their quality of care.
The goal of self-management programs is to empower individuals to deal with their disease and improve their quality of life. Participants of these programs build self-efficacy and confidence in their ability to manage their own health and the symptoms associated with their chronic condition and learn how to communicate effectively with their health care team.
The Online Chronic Disease Self-Management Program (Better Choices, Better Health®), is FREE to individuals across Ontario who are living with one or more chronic conditions such as diabetes, chronic pain, high blood pressure, cancer, heart or kidney conditions, etc. The program was developed at Stanford University and was designed to improve self-efficacy and confidence in the participants, and address issues that are most important to individuals living with chronic conditions.
