Living Well SE OntarioSelf-Management Program of Southeastern Ontario

About The Workshops

The Self-Management Program of Southeastern Ontario currently coordinates and supports three different Stanford self-management workshops:

These free, 6-week self-management workshops are led by two trained instructors who are often volunteers with chronic conditions themselves. Participants meet for 2.5 hours once a week to discuss such topics as:

  • Strategies to break the cycle of symptoms such as fatigue, pain, stress and anxiety
  • Developing action plans, process for goal setting, and problem solving
  • Appropriate exercise for strength, flexibility, and endurance
  • Appropriate use of medications
  • Communicating effectively with family, friends, and health care professionals
  • Nutrition

Each participant in the workshop will receive a copy of the book Living a Healthy Life with Chronic Conditions.

Who should take this workshop

People with chronic health conditions, their caregivers and family members can participate in self-management programs offered in communities throughout Southeastern Ontario at no cost.

Why take this workshop

The Stanford Self-management workshops provide the education and tools to help people develop self-management skills, empowering them to live healthier and better manage the day-to-day tasks of living with chronic conditions.

Learn how to:

  • Deal with the emotional, physical, and social aspects of living with a chronic condition.
  • Manage symptoms, treatment side-effects and medications.
  • Improve communication with health care team members.
  • Lead a healthy lifestyle, manage stress, make action plans and set goals.

Participants report improvement in their lifestyle and general health, as well as improved symptom management and better communication with their doctors. They also spend less time in the hospital compared to those who have not taken the workshop.

The workshops are a best practice, evidence-based program developed and evaluated by Stanford University. For more detailed information on the program evaluation, please visit Stanford’s website.

For more information on the differences between the three workshops, please visit the following description pages (Chronic Disease, Chronic Pain, Diabetes) or download the brochure.
